August 2023: According to new research, most persons with appendicitis who are given antibiotics instead of having their appendix removed perform well in the long run. The study discovered that among people who were originally treated merely with antibiotics rather than surgery, less than half experienced another incident or required appendix-related surgery years later. The most recent study focused on mild cases, which represent the majority of appendicitis patients who may not require surgical intervention and may potentially resolve spontaneously, avoiding unnecessary surgery and resulting in significant health care cost and hospital resource savings. The major takeaway here is that these long-term findings support the current solid evidence that surgery is not required for all patients with uncomplicated acute appendicitis, and that antibiotic therapy is viable and safe in the long run.

In 25 U.S. facilities, they conducted a pragmatic, nonblinded, noninferiority, randomized study comparing antibiotic therapy (10-day course) with appendectomy in patients with appendicitis. Secondary outcomes in the antibiotics group were appendectomy and complications through 90 days; analyses were prespecified in subgroups determined by the presence or absence of an appendicolith. On the basis of the findings of a common health-status assessment, antibiotics were noninferior to appendectomy for the treatment of appendicitis. By 90 days, roughly three out of every ten people in the antibiotics group had undergone appendectomy. Participants who had an appendicolith had a greater risk of appendectomy and complications than those who did not have an appendicolith.
